To participate in offsite activities, we require you to
fill a Waiver/Health Packet. Once it's filled out and on
file, it's good for the year. You can drop your
packet off at the church office M thru F between 9am and 4pm
or mail it to the attention of Cathie Behrens, 689 Hopmeadow
Street, Simsbury.
